It’s your lucky day! WAIT A MINUTE — YOU DIDN’T GIVE WRITTEN CONSENT FOR IMMTRAC2 AND YOU OR YOUR CHILD WERE ADDED ANYWAY?<\/p>\n

\ud83e\udd37\u200d\u2640\ufe0f Well, surely that couldn’t happen. The Texas Department of State Health Services website clearly says that STATE LAW REQUIRES YOUR WRITTEN CONSENT in order to add you or your minor child to the state’s registry that tracks immunization history. They wouldn’t BREAK THE LAW, right??<\/p>\n

\ud83d\ude21 Except they do. Over and over again, without consequence. TFVC receives several emails and messages every week from understandably irate Texas citizens who have had their personal information added to the registry either without their knowledge or IN SPITE OF having declined consent in writing. The letter pictured below was among one of the many messages we have received recently from the mother of a newborn who states she very intentionally declined consent for the baby to be added to the registry.<\/p>\n

\ud83d\ude21 Good thing requesting to be removed from the registry is so easy, right? WRONG AGAIN. You must download a removal request form and then MAIL OR FAX (apparently DSHS likes to party like it’s 1999) the request to the Texas Department of State Health Services in Austin. Links to do so will be provided at the end of this post. You might want to print out a few extra copies, because we also receive many messages of frustration from Texans all around the state who have yet to be removed despite sending multiple requests.<\/p>\n

\n\u2013 your phone number
\n\u2013 your date of birth
\n\u2013 your gender
\n\u2013 your race
\n\u2013 your medicaid number (if applicable)
\n\u2013 your social security number
\n\u2013 your immunization history
